ASSESSMENTS

Sage Prairie offers scheduled and walk-in assessments at all of its locations (from 9 am to 2 pm). A Comprehensive
Assessment is a clinical tool used to determine substance use disorder need based on the American Society of Addiction
Medicine’s guidelines. This tool can be used for placement in a facility, determine level of care, “rule in or rule out” substance use disorder, admit to treatment, or to see if a person would benefit from substance use disorder treatment care.
